In light of COVID-19 recommended procedures and the health and safety of our visitors and staff, in-office appointments can now be scheduled online. If you would like to book an in-person visit, please complete the form and be sure to select the following:

Our in-office hours are:
Tues-Thurs: 9:30 am – 3 pm (until further notice)
Sat-Sun: Emergencies only
We ask that all visitors arrive on time and bring along any relative information or documents. Should you need to cancel your appointment or you have any questions, please feel free to contact us.
